New Application
NSO Birth Certificate
Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory; for born 1950 & below, Negative Birth Record from NSO & Joint Birth Affidavit from Two Disinterested Persons if no record from NSO.

Original Valid ID
Original ID i.e. Senior Citizen’s ID, Voter’s ID, digitized government-issued IDs like SSS, PRC, BIR, Driver’s License, original school ID (for students only) AND original supporting documents indicating full name, date and place of birth and citizenship.
If your Birth Certificate was Late Registered:
- If registered on or before 2000: submit 1 original supporting document indicating full name, date and place of birth, and citizenship prior to the date of late registration.
- If registered on or after 2001: submit 2 original supporting documents indicating full name, date and place of birth, and citizenship prior to the date of late registration.
Certified True Copy of Birth Certificate
Certified True Copy (CTC) of BC issued by the Local Civil Registrar with dry seal if BC SECPA is unreadable.
Other Supporting Documents
Please bring the following if they are available.
- Marriage Contract
- Land Title
- Driver’s License
- Government Service Record
- School Form 137 or Transcript of Records with dry seal
- Other documents that show full name and birth details of applicant and/or citizenship
- Voter’s Registration Record from COMELEC Intramuros
- Baptismal Certificate with dry seal
- Seaman’s Book
- Income Tax Return (Old)
- NBI Clearance

Renewal
Brown Passport or issued prior to 01 May 1995
- Old passport and photocopy of passport pages 1,2,3, 4 (amendment). The pages showing latest Bureau of Immigration departure and arrival stamps.
- Original valid ID.

- Negative Birth Record from NSO if born 1950 & below and Joint Birth Affidavit from Two Disinterested Persons; and supporting document showing complete Middle Name

- For illegitimate minor, personal appearance of mother is required.
Machine Readable Passport (Maroon)
- Present passport and photocopy of pages 1,2,3 (amendment) inside and back cover.
- The pages showing latest Bureau of Immigration departure and arrival stamps.
- Photocopy of Birth Certificate for minor applicants.
- Original Birth Certificate from NSO in Security Paper (SECPA) if city/municipality & province on birth place is not specific (Example: NCR instead of San Juan, Metro Manila)
- For illegitimate minor, personal appearance of mother is required.
Other Supporting Documents
Please bring the following if they are available.

Lost Passports
- Same requirements as new passports plus:
- Notarized Affidavit of Explanation
- Police Report if passport is still valid
- Replacement fee of Php 200 if passport is still valid.

can i apply for a Phil. passport for my brother (65 yrs old) who lives in Negros Occidental online? i want him to visit me in Australia so he can enjoy the remaining days of his life.
I am scared that if he do it himself in DFA Cebu, he might be ripped off or robbed by bad people there. Do u know of a trusted travel agency who can do this for us?

I’m applying for passport renewal. I was a brown passport holder issued 1985. I already have the NSO Birth Certificate in security paper. Do I still have to secure the “Negative Birth Record from NSO if born 1950 & below & joint Birth Affidavit from two Disinterested Person; & supporting document showing complete Middle Name?” I was born November 24, 1950.

Pwede po ba mag-apply ng new passport pero ang id’s ko lang po ay student driver’s id,old school id,police clearabce,baptismal,form 137 yan lang po.pakisagot po salamat!

my wife applied for a passport but was rejected due to the fact that her documents were all new they asked her for old ids but she has non of them all she has are nbi clearance birth certificate marriage licence drivers licence etc but they are all new what else can we do?

Why is it that a baptismal Certificate is being required wherein fact in the list of requirements it is not present, also a Voters registration Record is being required to me at La Union wherein fact i had all the other list of requirements written in the list of requirements when applying a passport? can you help me please…

naku girl, yan din ang naging problema ko sa pamangkin ko, hiningan nila ako ng baptismal certificate eh di pa siya nabibinyagan, eh yung baby book naman di ko maipakita kasi iba pangalan niya dun bale 2 years siya nun.Ang ginawa namin pinabinyagan namin siya tapos bumalik kami sa dfa dala baptismal niya buti tinanggap naman…
depende yan sa natapat sayo na inspector, kasi yung katabing babae nung natapat sa akin na baklang inspector eh sinasabi na palusutin na ko kasi bata pa nman yung applicant pero hindi talaga pinalusot. pero kahit nahirapan ako eh at least may passport na pamangkin ko. Eh nung bumalik naman ako parang nagtaka yung inspector na babae kung bakit nareject yun nung una.. ewan ko sa kanila.
